Little Guardian of Louvinia Small, 46 Favre St Apt 1, Mattapan, MA 02126 requesting The City of Boston, Registrar, Registry of Births Marriages and Deaths, One City Hall Square, Room 213, Boston, MA 02201 for Amendment of my husband’s death certificate Registered # 7073, Marital Status: Divorced. I am requesting that my husband’s death certificate registered # 7073 be amended to read Marital Status: Married and Any Other Relief as this Honorable Court May Deem Just and Proper for the Reasons More Fully Described in the Petition.

You have the right to obtain a copy of the Petition from the Petitioner or at the Court. You have a right to object to this proceeding. To do so, you or your attorney must file a written appearance and objection at this Court before 10:00 a.m. on February 14, 2019.
