Senior Care Options (SCO) Case Manager Positions:

SCO Care Managers, under the general direction of the Team Manager, but working collaboratively with the SCO representative(s), the SCO Case Manager plays an integral part of the case management functioning of the agency. The SCO Case manager will provide initial assessment and ongoing assessments of the SCO client’s needs including eligibility for any relevant Boston Senior Home Care (BSHC) service and other community resources.

Boston Senior Home Care provides case management for the following plans:

• Boston Medical Center
• Health Plan
• Commonwealth Care
• Alliance
• Navicare
• Senior Whole Health
• Tufts
• United Health Care
