Case Managers, Administrative and Support Positons Available
Case Manager Positions:
Case Managers conduct home visits in the Greater Boston area to assess and evaluate elder’s eligibility for the State Home Care Program as well as other community resources.
Senior Care Options (SCO) Case Manager Positions:
Under the general direction of the Team Manager, but working collaboratively with the SCO representative(s), the SCO Case Manager plays an integral part of the case management functioning of the agency
Other Opportunities Include:
Health Care Registered Nurses, Long Term Support Coordinator; Development Coordinator; Senior Program Manager; Senior Information Specialist ; Transition/Discharge Planning Coordinator
