CASE MANAGERS

SMOC (South Middlesex Opportunity Council) has opportunities available for case managers in Hudson and Framingham, Ma. You will be providing case management services to families placed in family shelters, referred through DTA. Case Management will include assessment, service plan development, referrals to community-based resources, advocacy, aggressive housing search and crisis intervention. These services will be provided with the goal of assisting each family to attain and sustain a permanent housing placement. The caseload will include homeless families in emergency shelter and/or families who transition from shelter into permanent housing. These positions are FT w/benefits.
